Buscou, sobretudo, perceber nas subjetividades geradoras dos discursos, a influência do processo patologizante da sexualidade na geração de possíveis preconceitos e estigmas, como causadores de prejuízo à qualidade da atenção à saúde desta população. A 13ª Conferência Nacional de Saúde (2007) garantiu o acesso aos serviços de saúde, nos três níveis de atenção do SUS para população LGBT, considerando suas vulnerabilidades causadas por estigma e preconceito. A Política Nacional de Saúde Integral de LGBT (2011) tem como marca o reconhecimento de que a discriminação por orientação sexual e por identidade de gênero incide no processo de sofrimento e adoecimento decorrente do preconceito e do estigma social, e apresenta em uma de suas diretrizes a eliminação destes com foco ao combate das homofobias, inclusive a institucional. Mesmo após 28 anos da exclusão da homossexualidade do código internacional de doenças, a literatura tem apontado uma tendência à compreensão da condição ainda relacionada a patologias e grupos de risco para doenças sexualmente transmissíveis, configurando barreiras simbólicas ao acesso e qualidade da atenção. Estes são alguns dos marcos que potencializam a necessidade de compreender como a patologização e estigma poderão agir nas subjetividades de profissionais da saúde produzindo obstáculos à atenção a saúde integral a esta população e assim contribuir para a elaboração de novas estratégias para a educação permanente que mobilizem efetivamente tais estruturas subjetivas. Foram analisados as falas de 22 enfermeiros(as) e 21 médicos(as) nas regiões do Centro Oeste e Nordeste do País, através da Análise de Discurso. O resultado permitiu verificar que para os profissionais: a condição LGBT causa doença e transtornos mentais, estando também associada a comportamentos moralmente condenáveis; os princípios da equidade e igualdade são percebidos como antagônicos e nenhum dos(as) entrevistados(as) referiu ter sido profissionalmente formados para o atendimento da população LGBT. Tais resultados corroboram com nossa hipótese, de que a racionalidade biomédica, ela mesma, influenciada por uma Scientia Sexualis, tal como descrita por Foucault, contribui para um processo estigmatizador de deterioração das identidades, na forma descrita por Goffman, e se revela nos discursos dos profissionais e em suas práticas de cuidado. Text Any textual data included in the document <h3>INTRODUÇÃO</h3> O uso do conhecimento científico para melhorar o desempenho dos sistemas de saúde tem sido preconizado entre os países-membros da Organização Mundial da Saúde (OMS)1,2, a qual, em 2003, estabeleceu que suas próprias diretrizes deveriam ser baseadas nas melhores evidências3. No mesmo sentido, a Agenda de Saúde para as Américas (2008-2017) da Organização Pan-Americana da Saúde (OPAS) incluiu a aplicação de evidências científicas no fortalecimento da capacidade institucional e da liderança intersetorial na saúde entre suas linhas de ação4. Apesar de evidências científicas serem consideradas um elemento importante para o desenvolvimento de políticas públicas de saúde, o uso sistemático dos resultados de pesquisas científicas por tomadores de decisão ainda é incipiente5,6. Dentre as barreiras ao uso de evidências, destacam-se a pouca comunicação e colaboração entre pesquisadores e tomadores de decisão, o que representa importante limitação à incorporação do conhecimento científico ao processo de formulação e de implementação das políticas de saúde7. As limitações do diálogo entre a gestão e a academia têm origem nas diferenças das atividades, dos interesses e da forma de inserção dos profissionais nos processos relacionados à formulação, implementação e avaliação de políticas públicas de saúde8. Uma revisão sistemática da literatura indicou grande variação (7,0-40,0%) no uso direto de evidências científicas por parte dos tomadores de decisão6. A superação das dificuldades dos tomadores de decisão em usar o conhecimento científico e do baixo nível de interação entre os diferentes atores requer estratégias que considerem processos de tradução e de disseminação do conhecimento, adaptados a diferentes contextos institucionais e sociais6. Quando a questão é a tradução do conhecimento, uma análise da mídia impressa em países de baixa e média renda (PBMR) demonstrou que os países latino-americanos apresentaram a segunda mais baixa divulgação de evidências de pesquisas em saúde no mundo, ficando atrás apenas de países do Oriente Médio9,10. O mesmo estudo também revelou que os PBMR apresentaram o menor número de artigos a respeito da promoção de diálogo sobre políticas, informados por evidências científicas10. Promover o uso sistemático das evidências científicas nos processos decisórios representa um importante desafio para políticas de saúde no Brasil e no mundo. O objetivo deste artigo foi identificar e discutir estratégias para estimular o uso de evidências científicas no processo de tomada de decisão em políticas de saúde. A busca de evidências científicas foi realizada nos repositórios da Biblioteca Virtual em Saúde, Health Systems Evidence e PubMed. A ferramenta SUPPORT (Supporting Policy Relevant Reviews and Trials) foi usada na construção desse panorama. Sete revisões sistemáticas apontaram quatro principais estratégias para estimular o uso de evidências na tomada de decisão: 1) produzir e disseminar sínteses de evidência com linguagem adaptada a diferentes públicos; 2) estimular o uso do jornalismo e de outras formas de comunicação para ampliar a disseminação do conhecimento científico; 3) utilizar plataforma virtual on-line para disseminação do conhecimento científico; 4) promover a interação entre pesquisadores e tomadores de decisão. O Programa Saúde na Escola (PSE), implementado por meio do Decreto Presidencial n.º 6.286/2007, é uma política intersetorial entre os Ministérios da Saúde e da Educação, cujo objetivo é promover a atenção integral de prevenção, promoção e atenção à saúde de estudantes do ensino público básico no âmbito das escolas em parceria com as Unidades Básicas de Saúde da Família (UBS). Este artigo tem como objetivo caracterizar as ações de educação em saúde praticadas em três municípios da região Norte integrantes da pesquisa nacional denominada Projeto ARBOCONTROL, coordenada pela Faculdade de Ciências da Saúde da Universidade de Brasília (UnB), componente 3 Educação, Informação e Comunicação. Os dados foram obtidos por meio de entrevista semiestruturada aplicada a 34 professores. A prevenção aparece com maior frequência nos relatos mediante realização de palestras por parte das Unidades de Saúde, porém os professores não associam essa estratégia como uma ação integrada entre a escola e a unidade de saúde, decorrente do PSE, e sim como uma ajuda do “postinho”; sugerindo assim fragilidade no planejamento em conjunto de temáticas que devem ser trabalhadas lado a lado. Essa lacuna intersetorial é notada pelo não conhecimento e adesão das campanhas oficiais do Ministério da Saúde (MS), como instrumento pedagógico em sala de aula, e pelo fato de 85,29% dos professores entrevistados desconhecerem os materiais de campanha disponibilizados gratuitamente pelo MS. Aiken Description An account of the resource <p aling="" justify="">Many countries, including Israel, face health workforce challenges to meet the needs of their citizens, as chronic conditions increase. Provider shortages and geographical maldistribution are common. Increasing the contribution of nurse practitioners and other advanced practice nursing roles through task-shifting and expansion of scope-of-practice can improve access to care and result in greater workforce efficiency. Israel and many other countries are introducing reforms to expand nurses’ scope-of-practice. Recent international research offers three policy lessons for how countries just beginning to implement reforms could bypass policy barriers to implementation. First, there is substantial evidence on the equivalence in quality of care, patient safety and high consumer acceptance which should move policy debates from if to how to effectively implement new roles in practice. Second, regulatory and finance policies as well as accessible advanced education are essential to facilitate realignment of roles. Third, country experience suggests that advanced practice roles for nurses improve the attractiveness of nursing as a career thus contributing to solving nursing shortages rather than exacerbating them. The objective of this study is to describe transgender women’s and men’s labor market discrimination. A cross-sectional study was performed using data from 384 participants who identified as having a gender different than the one assigned at birth. Regarding their current work situations, 33.85% of transgender women and 45.16% of transgender men stated that they do not have a current occupation. A good extend reported having been denied a job opportunity because of their gender identity. Those who do get a job are exposed to discrimination and receive little or no social support from their colleagues and their bosses. The present research reinforces the necessity of policies to guarantee inclusion, and permanence of transgender people in the labor market." MÉTODOS: Foram realizados estudos de caso em Aracaju, Belo Horizonte, Florianópolis e Vitória com base em entrevistas semiestruturadas com gestores. Além disso, foi realizado um estudo transversal com questionários aplicados a amostras de profissionais e usuários da ESF. RESULTADOS: Em todos os municípios foram identificadas ações para fortalecer os serviços de atenção primária à saúde, com destaque para: aumento da oferta de atenção primária à saúde com diminuição das barreiras de acesso, estruturação dos serviços de atenção primária à saúde como porta de entrada do sistema, ampliação da resolutividade (apoio diagnóstico e terapêutico, promoção da interlocução entre equipamentos da rede de serviços para organizar o processo de trabalho, capacitação, supervisão) e articulação entre ações de vigilância e assistência. CONCLUSÕES: Os municípios investigados apresentam experiências consolidadas de reorganização do modelo assistencial com base em uma atenção primária à saúde fortalecida, com potencial para tornar-se coordenadora dos cuidados. Todavia, para efetivar a função de porta de entrada e serviço de uso regular são necessárias ações para equalizar o atendimento das demandas programada e espontânea, sendo que a última representa o maior desafio à organização do processo de trabalho das equipes. A conquista de apoio e legitimidade para a ESF é um tema pendente. Iniciativas para divulgar a ESF são necessárias entre a população, profissionais de todos os níveis e organizações da sociedade civil.
