The objective of this study is to describe transgender women’s and men’s labor market discrimination. A cross-sectional study was performed using data from 384 participants who identified as having a gender different than the one assigned at birth. Regarding their current work situations, 33.85% of transgender women and 45.16% of transgender men stated that they do not have a current occupation. A good extend reported having been denied a job opportunity because of their gender identity. Those who do get a job are exposed to discrimination and receive little or no social support from their colleagues and their bosses. Foi realizado um estudo de caso com abordagem qualitativa. Os participantes foram 11 enfermeiros de equipes da Estratégia Saúde da Família de áreas rurais do município de Campina Grande, Paraíba. A coleta de dados ocorreu no período de janeiro a março de 2017, por meio de entrevista, mediante uso de roteiro semiestruturado, e por meio de análise documental. Os documentos e as entrevistas foram submetidos à Análise de Conteúdo. Na análise dos documentos foram construídas duas categorias: "Políticas internacionais de saúde para área rural" e "Políticas nacionais de saúde para área rural". Das entrevistas emergiram quatro categorias: "Contexto de vida e saúde de populações atendidas pela Estratégia Saúde da Família de área rural: um lugar de população carente"; "Condições de trabalho de enfermeiros da Estratégia Saúde da Família de áreas rurais"; "Práticas cotidianas de enfermeiros em áreas rurais"; "Sentidos do trabalho para enfermeiros da Estratégia Saúde da Família de áreas rurais". No Brasil vem sendo criadas políticas para a redução das desigualdades em saúde dos diversos grupos de populações rurais do país, mas essas políticas não se efetivaram completamente. A primeira categoria revelou que a população rural tem condições de vida e saúde precárias; a maioria constituída de agricultores, aposentados, com elevado número de pessoas desempregadas. A Unidade de Saúde da Família constitui a única alternativa de acesso à saúde e há isolamento dessa população em relação ao uso de tecnologias de comunicação e informação. A segunda categoria descreve as dificuldades inerentes às condições de trabalho dos enfermeiros. As principais dificuldades foram: percurso longo percorrido pelo enfermeiro para acesso às Unidades de Saúde da Família rurais por meio de rodovia asfaltada e estrada de terra; cronograma de trabalho diferenciado em função das particularidades existentes do meio rural que influenciam o acesso ao serviço. A terceira categoria mostra que as práticas cotidianas dos enfermeiros estão focadas em grupos já conhecidos pelos protocolos ministeriais, como crianças, hipertensos, diabéticos, além de ações programáticas e de atendimento individual à demanda espontânea. Essas práticas são influenciadas pelas condições de trabalho, como transporte precário no deslocamento para as áreas rurais, falta de água nas unidades, o que é essencial para realizar os procedimentos e a assistência. A quarta categoria descreve os sentidos do trabalho para os enfermeiros com base nas dimensões dos sentidos do trabalho definidas por Morin; Tonelli e Pliopas, (2007), que são: individual, organizacional e social. Os enfermeiros identificam que o trabalho produz sentido positivo ao perceberem a satisfação pessoal no trabalho, crescimento e aprendizado, identificação com o trabalho, utilidade da atividade que realizam, vínculo com a população e a inserção e contribuição social do trabalho. Em contrapartida, a falta de plenitude no trabalho, em especial, decorrente das difíceis condições e da organização dos serviços, e a relação distante da gestão com os profissionais, foram os principais aspectos identificados pelos profissionais como perda de sentido para desenvolverem o trabalho. Conclui-se que o trabalho do enfermeiro da Estratégia Saúde da Família das áreas rurais estudadas possui uma dinâmica diferenciada, com 8 particularidades próprias da área rural e por dificuldades de infraestrutura, organização e operacional dos serviços, que afetam o desempenho do trabalho de enfermagem compatível com a Atenção Primária à Saúde. Aspectos esses que influenciam na atribuição de diferentes sentidos ao trabalho por este profissional. There are few studies addressing nurses' work in the rural context. This study aimed to investigate the satisfaction and difficulties of the work of primary health care (PHC) nurses in rural areas. Method: This is a case study with a qualitative approach, whose subject was the health units in the rural area of a large city in the state of Paraíba, Brazil, with Family Health Strategy teams. Eleven nurses working in rural areas participated in the study. Data were collected through interviews with semi-structured scripts and submitted to content analysis. Results: The study revealed that nurses working in rural areas have experienced job satisfaction with the recognition and gratitude of families, one of the major motivations for professionals interviewed. The nurses are committed and have adequate training in their area of activity. The working conditions, infrastructure, forms of access to the workplace and distance from the decision center are factors that stand out as difficulties of work. Conclusions: The nurses appreciate each other in their work, establish good relations with the population and work as a team despite the difficulties. The study reveals that nurses and the population experience isolation in the rural area as well as difficulty in accessing the workplace, in the case of practitioners, and in consolidating policies maintained in the urban area. It evidences the nurse as an essential element for PHC in the rural setting, which can make the difference in the care of populations often in the sideline of health services." Public health nursing has experienced challenges in balancing the paternalistic expert ideology and the empowerment participation ideology. A strong professional identity can legitimate nursing, and possibly influence the quality of nursing work. Narrations from practice can illuminate the nurses’ theoretical and practical knowledge and help illustrate their collective professional identity. Aim: To illuminate the meaning of public health nursing knowledge and professional identity in a continuously changing public health nursing practice. Method: A qualitative interview study with a purposeful sample of 23 Norwegian public health nurses was carried out. Data were analysed using phenomenological hermeneutics, a descriptive method inspired by Ricoeur’s philosophy of interpretation. Findings: Three themes emerged (i) Being a generalist: emphasising the need for generalised knowledge and using clinical judgement, (ii) Being one who empowers: focusing on resources and coping strategies, (iii) Being occupied with individual problem solving: focusing on individuals with special needs, using standardised techniques and protocols, and lacking specialised knowledge. Conclusion: Interpretation of the nurses’ stories illuminated their need for generalised evidence-based knowledge, but also the importance of using sound clinical judgement in a diverse complex practice, where service users need encouragement, support and expert advice. Time pressures can limit the nurses‘ involvement. Many had an individual problem-focus more than a primary prevention focus, in contrast to governmental regulations stating that Norwegian public health nurses should focus on health promotion and primary prevention. Public health nurses have a broad generalised knowledge of their special target group giving them a ‘specialist generalist’ role. Clarification of this role, in relation to jurisdictional borders, can create a strong identity at a time when healthcare policy promotes economic values, professional neutrality and increased collaboration. No estado de São Paulo, a incidência de doença falciforme é de um para quatro mil (1:4.000) nascidos vivos. Em situações de ausência ou diminuição da tensão de oxigênio, a HbS altera drasticamente a morfologia do eritrócito (forma de foice ou meia lua), causando vaso-oclusão e, consequentemente, isquemia, dor, necrose, disfunções, danos permanentes aos tecidos e órgãos, além de hemólise crônica. Devido às possíveis manifestações logo no primeiro ano de vida, é de suma importância o diagnóstico precoce na triagem neonatal. Dada sua complexidade, os cuidados globais à pessoa com doença falciforme são um grande desafio para gestores e profissionais de saúde ao envolver procedimentos que exigem uma rede de atenção regionalizada e integrada. Várias leis, decretos e portarias relacionadas à doença falciforme foram instituídos nos âmbitos federal, estadual e municipal. O diagnóstico precoce e medidas de profilaxia de infecções na infância têm melhorado a expectativa de vida dos indivíduos com doença falciforme. No entanto, o período da adolescência é de grande vulnerabilidade, uma vez que os pacientes enfrentam os desafios do desenvolvimento físico e psicológico próprios dessa fase, assim como as mudanças no seu acompanhamento pelos serviços de atenção pediátrica para os de atenção a adultos. Estudos indicam que a transição da atenção pediátrica para a adulta ocorre de forma abrupta, sem um preparo prévio do adolescente e seus familiares, fato que dificulta a manutenção do tratamento. Essa síntese teve como objetivo levantar opções para lidar com essa fase de transição da atenção pediátrica para a adulta de maneira a garantir a continuidade do cuidado aos adolescentes com doença falciforme e reduzir o abandono ou resistência ao tratamento. Opções para enfrentar o problema: Constatou-se que são escassas as revisões sistemáticas sobre doença falciforme, particularmente com relação à população de adolescentes. Por isso, as buscas por opções para políticas de saúde foram realizadas também para estudos em doenças crônicas.
